What is the 30-Day Form?

The 30-Day Form is one of Tarion's Statutory Warranty Forms. It's how you formally notify Tarion — and your builder — of any outstanding warranty items in your new home that haven't been resolved yet. You may submit only one 30-Day Form per home.

When do I need to submit it?

The 30-Day Form must be completed and submitted before the end of the first 30 days of possession of your home.

What information do I need?

  • Your home identification details (date of possession, vendor/builder number, enrolment number) — refer to your Certificate of Completion and Possession.
  • Your civic address and contact information.
  • A specific, itemized list of each outstanding warranty item, including the room/location and a description of each one.

How do I submit it?

You can submit the form online through Tarion's homeowner service, MyHome, by registering at tarion.com. You can also submit it in person, by mail or by courier to Tarion Warranty Corporation, 5160 Yonge Street, 12th Floor, Toronto, Ontario M2N 6L9. Whichever way you submit it, send a copy to your builder and keep a copy for yourself.
